Please evaluate\r\n\r\n\r\nComparison Study Date:\r\n5/6/2022\r\n\r\nTechnique:\r\nMultiplanar multisequence imaging of the brain was performed with and without contrast. \r\n\r\n\r\nFindings:\r\n\r\nThere is moderate opacification of the paranasal sinuses. There is bilateral mastoid fluid.\r\n\r\nThe brain is abnormal. Extensive areas of T2 prolongation are identified within the posterior upper cervical cord and brainstem, midbrain, periaqueductal gray, thalami, basal ganglia. Abnormal increased T2 signal with swelling is identified throughout \r\nmuch of the supratentorial cortex more noticeable in the parietal and occipital regions. This does affect the temporal and frontal lobes as well. Diffusion-weighted images are abnormal demonstrating restricted water motion along much of the cortical \r\nabnormalities near the gray-white matter junction. Restricted water motion within the basal ganglia and thalami is also noted to a degree. Involvement of some of the white matter is not entirely excluded as well.\r\n\r\nThe lateral ventricles are symmetric and not particularly dilated and similar to this morning CT. The cavum septum pellucidum seen on the 5/2/2020 examination is however no longer visualized suggesting a degree of mass effect. However, suprasellar \r\ncisterns remain patent with probably similar appearance of the medial temporal lobes and cerebellar tonsils when accounting for the differences technique.\r\n\r\nThe major intracranial flow-voids are grossly preserved.\r\n\r\nThe cerebellar tonsils are rounded appearing and above the level of foramen magnum and again similar. The adenohypophysis is somewhat thin appearing which may be within anatomic range. Neurohypophysis is orthotopic. Corpus callosum is present.\r\n\r\nSubcutaneous edema is questioned within the face.\r\n\r\nAs seen on the recent MRI, there does appear to be a degree of increased enhancement within the sulci bilaterally.
