FOR GENERAL QUESTIONS ABOUT QUEST\r\nDIAGNOSTICS GENETIC TESTING, PLEASE CALL THE GENE\r\nINFO LINE AT 1-866-GENE-INFO.-\r\n \r\nCarnitine, an amino acid synthesized in animal\r\ntissues from lysine and methionine by an\r\niron-ascorbate dependent pathway, functions as a\r\ncarrier of fatty acids across cell membranes.\r\nSerum carnitine analysis is useful in the\r\ndiagnosis and monitoring of patients with\r\ncarnitine deficiency (either primary or\r\nsecondary). Primary systemic carnitine deficiency\r\n(CDSP), also known as carnitine uptake defect\r\n(CUD) is an autosomal recessive disorder that\r\naffects carnitine uptake by cells \r\nand tissues\r\nthrough a defect in the plasma membrane carnitine\r\ntransporter (OCTN2, encoded by the SLC22A5 gene\r\nlocated on chromosome 5q31.1). The incidence of\r\nCDSP is approximately 1 in 50,000. The clinical\r\npresentation and age of onset of CDSP can vary,\r\nbut typical findings include hypoketotic\r\nhypoglycemia, hepatomegaly, cardiomyopathy,\r\nskeletal myopathy, and weakness. If diagnosed\r\nearly, treatment with carnitine can reverse many\r\nof the clinical symptoms. Secondary carnitine\r\ndeficiency occurs in some disease states, in\r\npatients on carnitine-poor diets, or secondary to\r\nmedications including valproic acid. It is also\r\nfound in a number of metabolic disorders affecting\r\nfatty acid and organic acid metabolism. In these\r\ndisorders, carnitine complexes with the\r\naccumulated substrate of the blocked metabolic\r\nstep, and the resulting acylcarnitines are\r\nexcreted in the urine, leading to a depletion of\r\ncarnitine in the patient. Elevated free carnitine\r\ncan occur in CPT-1 \r\ndeficiency or secondary to\r\ndietary carnitine supplementation, while increased\r\nesterified carnitine may indicate an underlying\r\nmetabolic defect. This test is not intended to\r\ndiagnose these disorders. All results should be\r\ninterpreted in the context of clinical findings,\r\nrelevant history, and other laboratory data.\r\n \r\nThis test was developed and its analytical\r\nperformance characteristics have been determined\r\nby Quest Diagnostics.
